Parish Registration

To enroll your child in our Faith Formation programs, your family must first be registered as parishioners of St. Mary's Parish.

If you are not yet registered, please complete our Parish Registration Form first. Once your parish registration has been submitted, return to this page to complete your child's Faith Formation registration.

Registration for Returning and New Students

Returning Students

If your child participated in our program last year, they have automatically been promoted to the next grade level. Please complete the registration form below to confirm your enrollment and update any contact information, such as email addresses, phone numbers, or emergency contacts.

If your child will not be returning this year, please notify the Faith Formation Office by calling 508-845-6341 or emailing [email protected].

New Students

Families enrolling for the first time should complete the registration form below. If your child has received faith formation instruction elsewhere, please contact the Faith Formation Office before registering.

Registration for Catholic School Students

Students attending Catholic school participate in our parish Faith Formation program only during their sacramental preparation years.

Grade 2 – First Reconciliation and First Holy Communion preparation
Grade 10 – Confirmation preparation (sessions are held from January through April)

Catholic school students do not participate in the Grade 9 Confirmation program.

Registration Fees

• Catechesis of the Good Shepherd - $50.00 per student
• Public School Students in Grades 1 - 10 - $100.00 per student, capped at $200.
• Catholic School Students in Grade 2 or Grade 10 - $50.00 per student
